Several key areas in modeling the cardiovascular and respiratory control systems are reviewed and examples are given which reflect the research state of the art in these areas. Attention is given to the interrelated issues of data collection, experimental design, and model application including model development and analysis. Examples are given of current clinical problems which can be examined via modeling, and important issues related to model adaptation to the clinical setting.

When in 2006 the Centers for Disease Control and Prevention issued revised recommendations for HIV testing in health care settings, vocal opponents charged that use of an ?opt-out? approach to presenting HIV testing to patients; the implementation of nontargeted, widespread HIV screening; the elimination of a separate signed consent; and the decoupling of required HIV prevention counseling from HIV testing are unethical. Here we undertake the first systematic ethical examination of the arguments both for and against the recommendations. Our examination (...) reveals that the ethical concerns raised by the critics predominately pertain not to ethically suspect elements of the recommendations themselves, but to suspicions that they will be implemented improperly. It has not been shown that the recommendations cannot be implemented properly. Here we show that in the United States the recommendations are morally justifiable and that safeguards or regulatory oversight may serve to ensure that the recommendations are properly implemented. Although the recent push toward sustainability is certainly generally a positive development in business and society, we can see many problems in the execution of the theory of sustainability. Where the triple bottom line calls on companies to weigh effects on stakeholders and the environment alongside profit, in practice in many cases, sustainability has been perverted to represent sustainable profits. In these cases, environmental impact and effects on people are only important insofar as they positively contribute to a firm‘s future (...) profits. It is not only practitioners who have often espoused this misappropriated view of sustainability but also academics have lent credibility to this view. In this work, we start by criticizing the often espoused current view of sustainability and remind academics of their responsibility to adapt a more critical view of this narrow focus. We provide examples that show how the current system of capitalism has resulted in outcomes for people and the environment that are patently unacceptable. Reasons are given as to why there is much hesitation to change the status quo. We then call on academics to reexamine what the role of businesses should be within society, what obligations business and corporations should have in society, and how we can encourage meaningful change that results in a better world for future generations. Background: In 2006, the Centers for Disease Control and Prevention (CDC) recommended three changes to HIV testing methods in US healthcare settings: (1) an opt-out approach, (2) removal of separate signed consent, and (3) optional HIV prevention counseling. These recommendations led to a public debate about their moral acceptability. Methods: We interviewed 25 members from the fields of US HIV advocacy, care, policy, and research about the ethical merits and demerits of the three changes to HIV testing methods. We performed (...) a qualitative analysis of the participant responses in the interviews and summarized the major themes. Results: In general, arguments in favor of the methods were based upon their ultimate contribution to increasing HIV testing and permitting the consequent benefits of identifying those who are HIV infected and linking them to further care. Conclusions: The prevailing theme of ethical concern focused on suspicions that the methods might not be properly implemented, and that further safeguards might be needed. (shrink)

The 2006 Centers for Disease Control and Prevention (CDC) revised recommendations for HIV testing in clinical settings contained seven specific changes to how health care facilities should provide HIV testing. These seven elements have been both supported and challenged in the lay and medical literature. Our first paper in BMC Medical Ethics presented an analysis of the three HIV testing procedural changes included in the recommendations. In this paper, we address the four remaining elements that concern HIV screening policy changes: (...) (1) nontargeted HIV screening, (2) making HIV screening similar to screening for other treatable conditions, (3) increasing HIV screening without assured additional funding for linkage to care, and (4) making patients bear the costs of increased HIV screening in health care settings. Fodor and katz criticize cavell's position on the relation between ordinary language philosophy and empirical investigations of ordinary language, In "must we mean what we say?," _inquiry, Volume 1, Pages 172-212, And "the availability of wittgenstein's later philosophy," "philosophical review", Volume 71, Pages 67-93. Cavell holds that disagreements between ordinary language philosophers over grammar and semantics are in no sense empirical. Fodor and katz show that ordinary language philosophers are engaged in empirical investigation. (staff).

Activation of gene transcription in vivo is accompanied by an alteration of chromatin structure. The specific binding of transcriptional activators disrupts nucleosomal arrays, suggesting that the primary steps leading to transcriptional initiation involve interactions between activators and chromatin. The affinity of transcription factors for nucleosomal DNA is determined by the location of recognition sequences within nucleosomes, and by the cooperative interactions of multiple proteins targeting binding sites contained within the same nucleosomes. In addition, two distinct types of enzymatic complexes facilitate (...) binding of transcription factors to nucleosomal DNA. These include type A histone acetyltransferases (e.g. GCN5/ADA transcriptional adaptor complex) and ATP‐driven molecular machines that disrupt histone‐DNA interactions (e.g. SWI/SNF and NURF complexes). These observations raise the important question of what happens to the histones during chromatin remodeling. We discuss evidence supporting the retention of histones at transcription factorbound sequences as well as two alternative pathways of histone loss from gene control elements upon transcription factor binding: histone octamer sliding and histone dissociation. The shift in corporate strategy, from vertical integration to strategic alliances, has developed hand in hand with the evolution of organizational structure, from the vertically integrated firm to the network organization. The result has been the elimination of boundaries, more flexible organizations, and a greater interaction among individuals and organizations. On the negative side, the specialization of firms on single areas of competence has resulted in the disaggregation of the value chain and in the disaggregation of ethical and legal responsibility. (...) To illustrate this point, the paper considers some cases, such as the case of the "beer girls" of Southeast Asia, who are used unethically by distributors to sell beer and liquor. To deal with the problem of the disaggregation of ethical responsibility, managers can use organizational culture and ethical values to control the performance of employees and of other organizations. Contemporary developments in business ethics also offer tools for dealing with the problem. For example, "global corporate citizenship," integrated social contracting theory, and stakeholder learning dialogues provide ways of integrating the interests of all stakeholders. The task is now to use these new approaches to create a governance process that incorporates the voices of all stakeholders, especially the voices of those stakeholders that have legitimate and urgent moral claims, but lack the power to establish those claims. The idea of a punitive God and/or the belief in a punitive God may have significant effects on group functioning. The troubling story of Ananias and Sapphira may be seen as a central cog in the cooperative coming together (...) of what became the Christian Church. The second messenger molecules cAMP and Ca2+ regulate a large number of eukaryotic cellular events. cAMP acts on protein kinases and Ca2+ works through a ubiquitous calcium‐binding protein, calmodulin. The two systems are not independent, however, but interact in several important fashions. These interactions, and, in particular, the modulation of the cAMP signal by two Ca2+/calmodulin‐regulated proteins, cyclic nucleotide phosphodiesterase and calcineurin, are described here.

This paper explores the difference between Connectionist proposals for cognitive a r c h i t e c t u r e a n d t h e s o r t s o f m o d e l s t hat have traditionally been assum e d i n c o g n i t i v e s c i e n c e . W e c l a i m t h a t t h (...) e m a j o r d i s t i n c t i o n i s t h a t , w h i l e b o t h Connectionist and Classical architectures postulate representational mental states, the latter but not the former are committed to a symbol-level of representation, or to a ‘language of thought’: i.e., to representational states that have combinatorial syntactic and semantic structure. Several arguments for combinatorial structure in mental representations are then reviewed. These include arguments based on the ‘systematicity’ of mental representation: i.e., on the fact that cognitive capacities always exhibit certain symmetries, so that the ability to entertain a given thought implies the ability to entertain thoughts with semantically related contents. We claim that such arguments make a powerful case that mind/brain architecture is not Connectionist at the cognitive level. We then consider the possibility that Connectionism may provide an account of the neural (or ‘abstract neurological’) structures in which Classical cognitive architecture is implemented. We survey a n u m b e r o f t h e s t a n d a r d a r g u m e n t s t h a t h a v e b e e n o f f e r e d i n f a v o r o f Connectionism, and conclude that they are coherent only on this interpretation. A certain metaphysical thesis about meaning that we'll call Informational Role Semantics (IRS) is accepted practically universally in linguistics, philosophy and the cognitive sciences: the meaning (or content, or `sense') of a linguistic expression1 is constituted, at least in part, by at least some of its inferential relations. This idea is hard to state precisely, both because notions like metaphysical constitution are moot and, more importantly, because different versions of IRS take different views on whether there are constituents of meaning (...) other than inferential role, and on which of the inferences an expression occurs in are meaning constitutive. Some of these issues will presently concern us; but for now it will do just to gesture towards such familiar claims as that: it's part and parcel of dog meaning dog2 that the inference from x is a dog to x is an animal is valid; it's part and parcel of boil meaning boil that the inference from x boiled y to y boiled is valid; it's part and parcel of kill meaning kill that the inference from x killed y to y died is valid; and so on. (See Cruse, Ch. 1 and passim.) IRS brings in its train a constellation of ancillary doctrines. Presumably, for example, if an inference is constitutive of the meaning of a word, then learning the word involves learning that the inference holds. If dog means dog because dog ---> animal is valid, then knowing that dog ---> animal is valid is part and parcel of knowing what the word dog means; and, similarly, learning that x boiled y ---> y boiled is part and parcel of learning what boil means, and so forth. IRS constrains grammatical theories. The semantic lexicon of a language is supposed to make explicit whatever one has to know to understand the lexical expressions of the language, so IRS implies that meaning constitutive inferences are part of the semantic lexical entries for items that have them. Lexical entries are thus typically complex objects (`bundles of inferences') according to standard interpretations of IRS.. (shrink)

Drug Safety Communications are used by the Food and Drug Administration to inform health care providers, patients, caregivers, and the general public about safety issues related to FDA-approved drugs. To assess patient knowledge of the messaging contained in DSCs related to the sleep aids zolpidem and eszopiclone, we conducted a large, cross-sectional patient survey of 1,982 commercially insured patients selected by stratified random sampling from the Optum Research Database who had filled at least two prescriptions for either zolpidem or eszopiclone (...) between July 1, 2012 and June 30, 2013. Among the 594 respondents, two-thirds reported hearing generally about drug safety information prior to starting a new drug, with the remaining one-third “rarely” or “never” hearing such information. Providers and pharmacists were primary sources of drug safety information. Two-thirds of zolpidem users and half of eszopiclone users reported having heard about the related DSC messages, ability to accurately identify the major factual messages was limited. Respondents reacted to new drug safety information about their sleep aids by reporting that they would want to learn about alternative ways to help them sleep and seek out more information about the safety of their specific sleeping pill. Opportunities may exist for the FDA to work with providers and pharmacies to help ensure the DSC information is more widely received and is more fully understood by those taking the affected medications. In this important new work, Jerry Day brings to light the need for an extensive reinterpretation of the mature philosophy of Eric Voegelin, based on Voegelin’s published and unpublished appreciation for nineteenth-century German philosopher F. W. J. Schelling. Schelling, whom Day maintains was one of the most important guides to Voegelin’s mature philosophy of consciousness and historiography, has been described as the father of several disparate movements and schools of continental philosophy—chief among them being “Hegelian” idealism and existentialism. This (...) characterization implies that Schelling was a scattered thinker with little or no appreciation for philosophy as a disciplined inquiry into the nature of human affairs. Voegelin was critical of this portrayal of Schelling. He argued that it lacked proper sensitivity for the impressive extent to which this giant of continental thought was able to rise above the “creed communities” of his time and recover the abiding concern of mature philosophers everywhere: the _philosophia perennis_. Those who claim that Schelling was scattered have failed, according to Voegelin, to appreciate the nonideological breadth of this great philosopher, misled by the splinter movements and schools that arose from mere fragments of his thought. In truth, Schelling founded no school and launched no movement. Instead, he reasoned with the disciplined integrity and wonder of a “spiritual realist.” Day argues that Voegelin was a fine interpreter of Schelling, particularly during the decisive years when the central orientation of Voegelin’s mature thought was beginning to take hold—between the writing of his _History of Political Ideas_ and its eventual transformation into _Order and History_. Day gathers an impressive array of evidence to interpret Voegelin’s little-known support for Schelling’s achievements, while offering detailed analyses and helpful summaries of a vast body of literature that has yet to be translated into English. Day’s partial agreement with Voegelin’s uncommon assessment of Schelling provides him with the point of departure that leads to one of this book’s most distinctive contributions to contemporary thought. It has the rare ability to help clear the way for philosophical realists to make peace with many of their contemporaries, giving them further grounds for accepting the strongest anthropological and psychological insights of recent continental philosophy, while helping them to avoid its tendencies toward nihilistic despair or fideistic historicism. By reading each philosopher through the eyes of the other, Day provides an analysis that will be illuminating for Voegelin scholars and Schelling scholars alike. The book will also appeal to readers with more general interests in the history and development of continental philosophy, political theory, and comparative religion over the past century. (shrink)

Ubiquitous computing is a new kind of computing where devices enhance everyday artefacts and open up previously inaccessible situations for data capture. ‘Technology paternalism’ has been suggested by Spiekermann and Pallas (Poiesis & Praxis: Int J Technol Assess Ethics Sci 4(1):6–18, 2006) as a concept to gauge the social and ethical impact of these new technologies. In this article we explore this concept in the specific setting of UK road maintenance and construction. Drawing on examples from our qualitative fieldwork we (...) suggest that cultural logics such as those reflected in paternalistic health and safety discourse are central in legitimising the introduction of ubiquitous computing technologies. As such, there is little doubt that paternalism plays an essential role in people’s reasoning about ubiquitous computing in this setting. We argue, however, that since discourses such as health and safety are used by everyone (including both managers and workers) in the organisation to further their own aims, technologies transcend purely paternalistic conceptualisations and instead become a focal point for ongoing struggles for control between those deploying and using them. This means that the benefits and costs of such new technologies become harder to define from an ethical and social perspective. (shrink)
